Records Office Requests
The Records Office's paper forms are requests you file in the SIS. Each one goes to the people who used to sign the paper form, and you are told the outcome either way.
Filing a request
- Go to Requests > Start Request
- Choose the request and fill in the form. Lists of terms, courses, sections and instructors are filled in from current data, so pick from them rather than typing
- Attach any document the form asks for (for example, a syllabus or the document behind a name change)
- Submit
Follow its progress from Requests > All Requests.
Which request to use
| Request | Use it to | Who decides |
|---|---|---|
| Special Request | Ask for an exception to an academic policy | The chair of your major recommends; the registrar or the Academic Standards Committee decides |
| Change of Name | Change your legal name | The Records Office |
| Transfer Course Approval | Check, before you take it, that a course at another school will transfer | The Records Office, then the chair of your major |
| Incomplete / In-Progress Grade | Ask for an I or IP grade in one of your sections | The section's instructor, then the Records Office |
| Directed Study | Take a catalog course one-to-one with an instructor | The instructor, the chair of the course's department, then the Records Office |
| Independent Study | Study a topic of your own with an instructor | The instructor, the chair of the department, then the Records Office |
| Late Add | Add a section after the add deadline | The instructor and your primary advisor, then the registrar or the Academic Standards Committee |
| Enrollment Verification Letter | Get an official letter confirming your enrollment | A registrar. Once it is approved, the letter is emailed to you (and to up to 10 other addresses you list, separated by commas), and you can download it from the request |
Where your advisor approves, it is your primary advisor. If you have none on file, the registrar decides that step instead.
Changing your name
While Colleague is still the system of record for names, a name change also has to be made there, or the nightly update restores the old name. If your old name reappears after an approved request, contact the Records Office.
